We've play-tested my paladin through five levels now. I've bumped the hit dice down to 1d10 (the suggestion to do so was a good one!) and made the smite table more generous by +1. Also on a successful smite the paladin now gets +2 to his MDoA result (just for purposes of MDoA affects.) The couple times that fumbles on special abilities have come up in play I've thrown out the roll on the divine disapproval table in favor of an ad-hoc disapproval result from the DM.
