Companies have to submit their games/supplements for consideration; if DCC wasn't submitted, that would explain its lack of appearance.

The Crawler's Companion is rightfully nominated.

Self-nominate in the sense that one fills out a form to say "Hey, I have something that was created this year that you can consider." Then the judges go through the entire list of submitted items and nominate the stuff that will actually be voted on. (The list of 'nominees'.)

So not everything that is submitted for consideration is nominated. But if I understand correctly (and I have no juicy insider info), none of the awesome GG catalog from the previous year was submitted for consideration. The motivation behind that decision is apparently a good story. ;)

(In the past Goodman Games has won many Ennie Awards.)

For books published in the physical sense, you actually have to send in multiple copies (one for each judge). At least, when I ended up nominated for Best System that was certainly the case.

Publishers don't really self-nominate. The process:

1) Publishers send multiple copies so the judges can be sent them and read and debate them.
2) The Judges decide among themselves on the handful of entries they consider the best in each category out of the HUGE swathe of books, etc. they're sent. They get sent a LOT, so by far the vast majority do not end up in the final category lists as selected by the judges. No publisher self-nomination; the only thing the publisher can do is submit materials in the hopes that they'll make the final shortlists the judges decide on.
3) The public then vote on their favourites of the judges' nominations in each category. Suffice to say, the size and passion of a given system's/publisher's fanbase is THE deciding factor (hence why the likes of Paizo tend to win their nominations in the public vote).

Nice! For everyone else, here's my post from last year, if that helps.
A couple that come to mind that need adding to the list would be http://www.mysticbull.com/ & Dragon's Hoard Publishing.

I also see that in addition to Purple Sorcerer goodness getting a nomination, our friends at Crawl! are nominated in the Fan Favorite category. Both are well deserved.
