When I had a group that cared, this is what I used (though I used a spreadsheet). There's a little bit of campaign artifact there (month names), but otherwise, the two files include a "Month Tracker" and a "Round, Turn, Hour" tracker. This campaign we tracked a lot of stuff in town (that essentially happened off camera), and the months actually mattered to the campaign. Current campaign, I don't use this same system, but it seemed to work -- I did the PDF's because I stopped using a computer at the table -- so it was easier to jot notes on paper and then update our official "campaign record." Tedious, but it's what everyone wanted.
